CLEMENTS INTERNATIONAL TERM LIFE APPLICATION PROCESS

1. We will need the application and a copy of your passport (first two pages – the one with your picture)
   1. Fill out the application from our web page at: http://www.onlineglobalhealthinsurance.com/clements-term-life
   2. May start up to 60 days before the requested effective date. Premiums must be paid prior to the effective date
   3. The signature date and the requested effective date may not be more than 90 days apart
2. Once we have this information we will submit the application for underwriter review
3. Depending on the answers provided to the medical questions on the application, the underwriter may require additional information such as blood test including HIV, physical, and or urinalysis tests. If you have had any of these exams performed over the last 90 days, we can use those results.
4. The effective date:
   1. For U.S. citizens - starts the day you leave the U.S. / The day you are outside of the U.S., or as we like to call it “wheels up.”
   2. Non U.S. citizens may also apply. The effective date is the day you are outside of your country of citizenship, or as we like to call it “wheels up.”
5. Your policy:
   1. Your policy book will be mailed to the mailing address placed on the application
   2. Once accepted, you will receive and email with your policy conformation
